About this Paper

We have designed and developed a computer program to provide clinical decision support to physicians managing patients with type 2 diabetes.  We included 8 classes of medications: biguanide, DPP-4 inhibitor, GLP-1 agonist, secretatgogue, TZD, AGI, and basal insulin.  There are 68 treatment regimens including 8 mono-, 26 dual-, 31 triple-, and 3 quadruple-therapy in addition to lifestyle modification. The programs is integrated with a previously described Comprehensive Diabetes Management Program (CDMP) and interfaces with an EMR and laboratory system. The program is based on demographic data, allergies, contraindications, etc. It checks the validity and quality of the data. The program provides recommendations to increase or reduce dosages, add or discontinue medications, and (in prototype versions) provide explanations.
