About this Paper
The Comprehensive Diabetes Management Program (CDMP) is a web-based diabetes medical informatics tool developed by a consortium of researchers, physicians, and educators specializing in diabetes management, aiming to provide an interactive clinical tool for care managers to efficiently coordinate care for diabetic patients and encourage behavior changes that lead to improved outcomes. The CDMP features two distinct user interfaces: one for providers and care managers (CDMP) and another for direct patient access through the CDMP patient access portal (DMEverywhere). Currently, the CDMP is utilized in 12 organizations, with data being collected for studies on clinical efficacy and cost efficiency.
